Kya Miller LICSW, LCSW - Evaluation for ESA Letter
Kya Miller, LICSW, LSCW was the clinician assigned to my ESA evaluation. I accidentally entered my phone number incorrectly (when I registered on Pettable). As a result of that, I missed my appointment time. Kya reached out through the patient portal to let me know there was an issue with my contact information.
