We understand that patients want a clear picture of all costs involved before beginning the process. The fee paid to QuickMedCards covers the medical evaluation and, if approved, the physician certification. Arizona’s application fee is charged and collected separately by the Arizona Department of Health Services and is not paid to or retained by QuickMedCards. Our website explains that government fees must be paid directly by the patient, and this is also included in the application instructions we send after the appointment.
