Turns out, they are only filling out the form for you to take yourself to your local Department of Vital Records/Statistics. You just need to contact them directly and fill it out yourself for no extra charge.

Hopefully it has really fast shipping. It also says there’s a warranty. The exact reason I am in need of a new birth certificate now is because of a mishap in the mail. Warranty is good news. I put in simple info like my parent’s names. That’s all. Where I want to ship it to as well.
