incomplete instructions
There is no option, under relationship to the deceased that includes a family member not "directly" related. I am both the deceased's first cousin AND her executor so I qualify as the person in charge of the disposition on the death certificate. But my application has been voided. How do I prove I am an "entitled person?" The website offers no guidance with this.

Beware of Vital Forms Diect - scam in sheeps cloth.
Beware of this company. Their advertising is deceptive and a scam. My eyesight leaves me half blind. Google puts Vital Forms Direct at the top. I rushed through the pages thinking I was using the company called VitalChek. When I went to pay the $99 fee (which I wondered why so much) the credit card company denied the transaction stating possible fraud. I used another card and it to was denied - possible fraud was reason. Finally used a third card and it went through. What I paid for was a six page form they later had be download, complete, and then take to be notarized. This is not a requirement in California. So the notary will be at least $20,
and I must send and additional fee 0f $19.00 to the California Department of Public Health. That department has nothing to do with marriage records. Next I have to pay at least $7.00 to send the form to Sacramento. This is completely bogus. The individual county where the marriage happened is where the certificate comes from. Vital Forms Direct is not direct. This is a scam company preying on old, half-blind people like myself. Please do not use this company. Go to VitalChek.com
for quick and honest service. $48 was the total cost, no notary, no forms to fill out, and it comes directly from the county registrar. I filed a dispute with Mastercard but Vital Direct will say all the information was there. I will have to eat the $99.00. Shame on Vital Forms Direct.

I'm not sure if I'm getting a digital copy or a certificate. Last time I did this - I was shown all of the other people still alive with my sister's name.
