1. The service is advertised as free. It is only "free" if you sign up for a trial, which is not free. 2. The template was not formatted properly, dividing a boxed entry across pages, which renders it useless. I tried exporting to Word and reformatting, which also did not work. Ended up downloading the free template from the state, which is less useful but works.

Wanted to try this service. It was easy to access the form I was interested in. The form was easy to use. But the monthly fee was too much for a casual user. This would be better served for a company than a single user. Cancelling was easier than I feared.
